PIP is about how your condition affects daily living and mobility, not the diagnosis alone. Documents can give context, but your daily difficulties and real-life examples still matter.

Diagnosis or condition letters

Letters that name or explain a diagnosis or health condition.

Medication list or prescriptions

A current medication list, repeat prescription, or notes about side effects.

Appointment letters

Letters showing appointments with health professionals or services.

Treatment or therapy notes

Notes about therapy, treatment, care, or support you receive.

Care or support plans

Plans that explain what help you receive or what support has been agreed.

Letters from health professionals

Letters from a GP, consultant, therapist, nurse, support worker or other professional.

Notes from school, college, workplace or support workers

Useful if someone else has recorded how your condition affects you.

Your own diary notes or real-life examples

Examples from daily life can help show what happens, how often, and what support you need.
